Support Coordination. We help you understand and make the most of your NDIS plan. We connect you to the supports and services best suited to your unique needs. Therapy? Support Workers? Assistive Technology? Home and Living Solutions? Community Access? No cookie-cutter solutions here: we work with you to grow a team specific to you and your goals. Our Support Coordinators commit to helping you build your capacity to run your own show – and we will be there beside you as much or as little as you need.
Psychosocial Recovery Coaching We provide specialised guidance to those with disabilities as a result of mental health challenges. Our focus is on supported recovery and capacity-building. We start with where you are at and then build a team of supports to help you on your recovery pathway. This will include a mixture of mainstream health supports (like your local mental health service, psychiatrist, GP) and NDIS funded supports. Our Psychosocial Recovery coaches have both lived experience of mental health conditions and professional training in the field. We will celebrate your every success, but we understand this path can be two steps forward, one step back, so we plan for the fluctuating impact of mental health conditions.
Registered Nursing Our qualified Nurses deliver nursing care and disability-related health supports, and ensure your needs are managed with excellence and compassion. Additionally, Jacaranda Living Supports provide specialist continence care, addressing sensitive needs with dignity and professionalism. Each service is rendered with a focus on personalisation and quality, ensuring participants receive the best possible care and support.
Support Workers We handpick our Support Workers to suit you and your goals. We will ask you exactly what you want (and don’t want!) from a Support Worker. At Jacaranda, we know what it is like having Support Workers in your home and your life. We know how great it is to have the right person, and how difficult it is when it isn’t a good fit. Finding someone reliable, kind and a match for your needs and interests can feel like looking for a unicorn. We pride ourselves on being expert unicorn hunters!
